Mechanical Modelling for Peyronie’s Disease

Many patients when they first get diagnosed with Peyronie’s disease try some kind of penile stretching or modelling (during an erection, trying to straighten the penis).

There is no strong evidence that mechanical modelling on its own can help reduce penile curvature and plaque size. However, mechanical modelling is an integral part of the first stage of treatment (see here) when combined with Xiapex injections, so as to break the plaques and also very important to follow post-surgically, so as to prevent penile retraction and further loss of length, reduce any residual curvature and also increase penile blood flow that assists with the post-surgical healing process.

Various mechanical modelling protocols have been proposed but based on our experience we advise our patients to follow this regime below (after Xiapex injections or post-surgically):

Vacuum pump therapy twice daily for 10 min each

  • Gradually pump penis (making sure you don’t over pump once it’s erect)
  • Leave erection on for 30 sec
  • Deflate and start again (5times)

Penile stretching exercises – each time you go to the toilet, gently and gradually stretch the penis until maximum (10 times). Alternatively, a penile stretching device can be used for 4-8 hours (the longer the better) a day some patients find this difficult.

Penile self modelling – each time you have an erection, gently try to straighten the penis.

Your doctor will show you exactly how to apply mechanical self-modelling and will instruct you when to start after your Xiapex injections or surgery.
